The answer, first. Synthetic gonadotrophin-releasing hormone, identical to the ten-amino-acid hypothalamic hormone that drives the pituitary to release LH and FSH. The strongest evidence is for diagnostic test of pituitary gonadotrophin reserve, graded A — approval or replicated trials.
Approved in the United States as Factrel for evaluating the pituitary's capacity to release LH, application NDA018123 1.
Marketing status is now discontinued. The stimulation test still separates gonadotrophin deficiency from constitutional delay 7. The other use — keeping testicular function during testosterone therapy — grades D or E: animal or mechanism data with no controlled human outcome.

How it works

What it does not do

- A twice-weekly injection restores the axis. Nothing published supports bolus dosing on that schedule. The approved fertility product delivered a pulse every 90 minutes by pump 110.
- Interchangeable with hCG on testosterone therapy. hCG acts at the testis, gonadorelin at the pituitary, which exogenous testosterone has already suppressed. No head-to-head trial exists.
- Raises testosterone in healthy men. Never tested. Every trial enrolled patients with a diagnosed hypothalamic or pituitary defect 239.

What is Gonadorelin used for?
Its approved uses are a diagnostic test of pituitary function, as Factrel, and pulsatile ovulation induction in hypothalamic amenorrhoea, as Lutrepulse 1. Both US products are discontinued.
It also induces spermatogenesis in hypogonadotropic hypogonadism when delivered by pump 39. In men's clinics it is prescribed alongside testosterone to preserve testicular function, a use with no published trial.
How is Gonadorelin used?
Descriptively. The approved fertility use is a wearable pump delivering a pulse roughly every 90 minutes, because that is what the pituitary responds to 110. In men, pulsatile therapy ran 5 to 25 nanograms per kilogram per pulse, titrated to testosterone 3.
The diagnostic use is one dose from a 0.1, 0.2 or 0.5 mg vial 1. Half-life is 5.5 to 8 minutes 5.
How long does Gonadorelin take to work?
For the diagnostic test, LH rises within 30 minutes 7. For fertility, spermatogenesis on pump therapy appeared at a median 6 months in one comparison 4.
Men with no prior pubertal development had not normalised testicular volume by 24 months 3. For the testosterone-adjunct use, no timeline exists, because no trial has measured one.
Is Gonadorelin an approved drug?
Yes, for two specific indications, and both US products are now discontinued 1. An approval attaches to a named indication, a named dose and a manufactured product.
It does not extend to a compounded vial, to an off-label goal, or to a twice-weekly schedule worked out on a forum. Under the World Anti-Doping Code it is prohibited in males at all times, class S2.2.1 6.
